Who home visits are for
- Patients who are homebound or have trouble getting out of the house
- People recovering from an illness, surgery or hospital stay
- Older adults and folks without reliable transportation
- Caregivers who need help keeping a loved one healthy at home
What happens at a home visit
Our nurse checks in on how you're doing, takes vital signs like blood pressure, reviews your medicines and stays in touch with your provider at Minuteman Medicine. Your provider can follow up by telehealth when that makes sense.
