Photo ID
Insurance card (Medicaid, Medicare, or private)
All your medicines and vitamins, or a list
Names of your other doctors
Any recent test results or records

Insurance

We accept all insurance plans, including Kentucky Medicaid plans, Medicare and private insurance. We do not accept workers' compensation. Self-pay patients are welcome, so call us at 606-949-1006 with any questions.
